Free official game piece, and official rules, may be obtained, while supplies last, by sending a self-addressed, stamped envelope along with a 3 1/2” x 5” piece of paper hand-printed with your name, complete address and age to: ONE-A-DAY® Kids Scooby-Doo™ Game Piece Request, PO Box 1374, Grand Rapids, MN 55745-1374. WA and VT residents may omit return postage. Limit one game piece request; limit one request per outer mailing envelope per day. Requests must be postmarked by December 1, 2004. No mechanical reproductions allowed.

2. HOW TO PLAY. Open the package carefully and look at the game piece printed on the inside panel. If Scooby-Doo symbol appears with the word “Congratulations!” you are a potential winner of a Grand Prize – a trip to Hollywood, California. See specific details of prize below.

3. PRIZE. One Grand Prize – a trip to Hollywood, CA for four people. Grand Prize valued at $8,000. Actual value of prize may vary depending on point of departure and fare fluctuations. Trip component includes: round trip coach class airfare for four from the major airport nearest the winner’s home, to Los Angeles, CA; two double rooms, double occupancy, deluxe hotel accommodations for four, for six days and five nights, and a medium size rental car. Car rental restricted to an eligible driver. Trip also includes Warner Brothers’ VIP tour, tour of animation facility including a Scooby-Doo costume character appearance, lunch and an animation cell of the winner will be created by an animator. Animation package is subject to availability. Minors must travel accompanied by a parent or legal guardian. Entire trip and all trip components subject to availability. Meals, gratuities, and all other expenses not specified herein are not included. Trip must be completed by August 31, 2005. Unclaimed prize will not be awarded. Sponsor makes no express or implied warranties regarding prize.

4. HOW TO REDEEM PRIZE. To redeem prize, sign your full legal signature in ink across the face of the official game piece. Then mail your official game piece (keep a copy for your records) via certified mail, return receipt requested, along with a 3 1/2” x 5” piece of paper on which you have printed your name, address, city, state, zip code, age and telephone number to: ONE-A-DAY® Kids Scooby-Doo™, P.O. Box 1223, Grand Rapids, MN 55745-1223. Prize claim must be postmarked by December 21, 2004 and received by December 31, 2004.

5. ELIGIBILITY. Game open only to legal residents of the continental USA over 18 years of age as of December 31, 2004 . Employees of Bayer HealthCare LLC, its distributors, wholesalers, affiliates, subsidiaries, advertising, promotion or other agencies, media partners, retailers, and members of the immediate families of each are not eligible to participate.

6. OTHER CONDITIONS. Game materials are automatically void if counterfeited, mutilated, forged, altered, or tampered with in any way, if illegible, mechanically reproduced, obtained outside authorized legitimate channels or if they contain printing, production, typographical, mechanical, or other errors. Bayer HealthCare LLC is not responsible for lost, late, illegible, incomplete, damaged, mutilated, misdirected, or postage due mail, requests, prize claims or entries. All official game pieces are subject to verification by Arrowhead Promotions, an independent judging organization before any prize is awarded. Decisions and interpretations are final, binding and conclusive in all matters. Liability for game pieces containing printing or other errors is limited to replacement with another game piece while supplies last. Only the intended prize as outlined in these Official Rules will be awarded. Potential winner will be notified of prize verification by mail within 10 days and will be required to sign and return affidavits of eligibility and liability/publicity releases within 14 days of notification of prize. Noncompliance within this time period will result in disqualification. Guests of trip winner will be required to sign and return liability/publicity releases prior to travel. Taxes on prize are winner’s responsibility. No correspondence will be acknowledged or returned. Prize cannot be transferred, substituted, or redeemed for cash value except as provided herein. By accepting prize, winner consents, where lawful, to use name and/or likeness, and/or voice of winner in any manner by Bayer HealthCare LLC, its advertising, promotion, or public relations agencies, without further compensation. By accepting prize, winners agree that Bayer HealthCare LLC, its parents, divisions, subsidiaries, affiliates, advertising, promotion and public relation agencies of each shall have no liability whatsoever, for any injuries or damage incurred in whole or part from acceptance, possession, use or misuse of any prize, or participation in this promotion.
